* For duplicate bookings, specify details of duplicate bookings under COMMENTS

* For Medical, Death issues or Billing errors, please attach relevant supporting documents. 

* For Travel Insurance, please attach your insurance policy here. 

* For strict compliance, refund applications for tickets purchased via Travel Agent/B2B Portal will have to be submitted to the same party for processing. 

* Refund requests exceeding 06 months from the date of purchase require Savings Account details in order to facilitate the refund processing.
